Baseball 4x Regional Champions!

Congrats to the varsity Mustang Baseball team who defeated Baptist Prep for the Regional Championship on Saturday! They’ll be the #1 seed in the State Tournament this week.

Free Athletic Physicals
Free athletic physicals are scheduled for Tuesday, May 7th at the Secondary Campus! There is no cost or charge to your insurance, so if you choose, you will still have the option of using your yearly free physical with your family doctor.  Please read carefully for further instructions to allow this evening to go smoothly.

6th – 8th grade athletes should arrive at 6:00.
9th – 11th grade athletes should arrive at 6:30.

Check-in will be in the atrium.

Print and complete the following forms BEFORE you arrive
Medical history form (completed and SIGNED by parent or guardian)
Martin Orthopedics HIPPA form
CAC medical consent for
Physical Examination form, this will be filled out by our providers (leave blank)

These required forms can be found here on the CAC website. Again, all 4 forms must be printed, and the first 3 MUST be filled out and signed by a parent in order for them to proceed to the exam. Parents are welcomed to attend with their child, but not required if all paperwork is completed beforehand.
