The deadline to complete the Opt-Out Form for the upcoming school year is NOON on FEBRUARY 1.
By submitting the Opt-Out Form the parent/guardian informs the Admissions Office of their intent not to enroll their student(s) at CAC the next school year. Once the Opt-Out Form is submitted, the student’s spot will be opened up for another student.
If the parent/guardian chooses to re-enroll their student after submitting the Opt-Out Form, they must contact Chad Tappe (firstname.lastname@example.org) in the Admissions Office to proceed with the re-enrollment process. The re-enrollment fee is $450.
Complete the form below prior to NOON on FEBRUARY 1st to begin the opt-out process.
