Please complete this form if your family would like to participate in the remote learning option for the third quarter beginning January 5, 2021 OR if your family participated in permanent remote learning for the second quarter of the fall semester (October – December).

In addition to completing the form, all families interested in remote learning must communicate with their campus principal for approval and instructions.
